Ruling on endometriosis causing periods to last 11 days instead of 7 — is it istihadah?

Question
Answer
Istihada (Irregular Bleeding) – Ruling on Prolonged Menses Due to Endometriosis
Wa alaykum as-salam wa rahmatullahi wa barakatuh.
Short Answer
If your normal period is 7 days and endometriosis causes bleeding to continue beyond that, then the first 7 days are menstruation (hayd) and the extra days are istihadah (irregular bleeding). You should make ghusl after the 7th day and resume praying — even if bleeding continues. You do not wait until the 11th day.
Detailed Ruling
1. The Principle: Menstruation Has a Known Duration
The Prophet ﷺ said to a woman experiencing prolonged bleeding:
«تَحَيَّضِي سِتًّا أَوْ سَبْعًا، ثُمَّ اغْتَسِلِي، وَصَلِّي»
"Consider yourself menstruating for six or seven days, then perform ghusl and pray."
— Sunan Abu Dawud (287), Sunan al-Tirmidhi (128), Sunan Ibn Majah (626) — classified hasan/sahih by Shaykh al-Albani in Sahih Abi Dawud
This hadith establishes that a woman returns to her known habitual cycle (ʿādah) to distinguish hayd from istihadah.
2. Applying It to Your Case
- Your normal cycle: 7 days
- Current bleeding: 11 days (due to endometriosis)
Ruling:
- Days 1–7 → Hayd (menstruation) — no salah, no fasting, no intimacy
- Days 8–11 (and beyond) → Istihadah — you are pure (tahir); you must pray, fast, and may resume marital relations
This is the position of Shaykh al-Islam Ibn Taymiyyah (Majmu' al-Fatawa, 19/237), Ibn al-Qayyim (Zad al-Ma'ad), Shaykh Ibn Baz (Majmu' Fatawa, 10/159), and Shaykh Ibn Uthaymeen (Ash-Sharh al-Mumti', 1/387).
3. When to Perform Ghusl
You should perform ghusl after the 7th day ends — i.e., before Fajr on the 8th day (or whenever the 7th day completes according to your cycle count).
Evidence: The Prophet ﷺ commanded the woman with istihadah to make ghusl once the days of her habitual menses ended, then pray — even if blood was still flowing.
«ثُمَّ اغْتَسِلِي، وَصَلِّي»
"Then perform ghusl and pray."
So yes — ghusl before Fajr on the 8th day is correct.
Practical Steps for You
| Day | Status | Action | |-----|--------|--------| | 1–7 | Hayd | No salah, no fasting, no intimacy | | 8 onward | Istihadah | Ghusl, then pray, fast, etc. |
For Istihadah Bleeding:
- Perform ghusl once when hayd ends (before Fajr of day 8).
- For each prayer, do the following:
- Wash the private part
- Apply a pad/cotton to contain the blood
- Make wudu for each prayer time (after it enters)
- Pray immediately — wudu remains valid until the next prayer time enters, even if blood flows
- You may fast in Ramadan during istihadah days.
- You may pray, recite Qur'an, and have intimacy with your husband.
Evidence for wudu per prayer:
«ثُمَّ تَوَضَّئِي لِكُلِّ صَلَاةٍ»
"Then perform wudu for every prayer."
— Sahih al-Bukhari (228)
Important Notes
- If your cycle changes permanently (e.g., it consistently becomes 11 days for several months), then the ruling may change — consult a knowledgeable person.
- If you cannot distinguish due to continuous bleeding, return to your last known habit (7 days).
- Endometriosis is a medical condition — seek treatment, but it does not change the fiqh ruling above.
- Do not delay ghusl thinking you must wait for bleeding to stop completely. Istihadah bleeding does not prevent prayer.
